Perimenopause means "around menopause" and refers to when your body naturally transitions to menopause, marking the end of the reproductive years. Perimenopause is also called the menopausal transition and typically lasts 4-8 years.


During the midlife perimenopause transition, hormonal changes begin and you may experience menstrual irregularity. Changes to menses can start as early as your 30s, but perimenopause most often begins between the age of 40 and 44.

Hormones are responsible for transmitting signals through the bloodstream and tissues. These chemical messengers influence a variety of processes, including metabolism, tissue growth, organ function, cognition, and mood.


All bodies use estrogen, progesterone, and testosterone, among other hormones. Levels of estrogen in your body rise and fall unevenly during perimenopause and progesterone slowly declines. During menopause, changes in your body cause menstrual cycles to lengthen or shorten, become heavier or lighter, and ovulation may not occur during some cycles.  


Physical Changes in Perimenopause

  • bone loss begins due to decreasing estrogen
  • increase in LDL (bad cholesterol) due to decreasing estrogen
  • decrease in HDL (good cholesterol) due to decreasing estrogen
  • thinning of the vulva and vaginal tissue due to decreasing estrogen 


Symptoms of Perimenopause

  • hot flashes
  • sleep problems
  • night sweats
  • vaginal dryness
  • pain with sex
  • mood changes
  • decreased sexual arousal
  • decreased desire
  • hair loss
  • weight gain

Menopause is the normal conclusion of menstrual cycles and occurs after the erratic hormonal changes of perimenopause. After 12 months of no menstrual periods (without other causes), menopause can be confirmed. 


Menopause most often occurs between the ages of 45 and 55, with an average age of 51, and is a single point in time marking the end of menstruation. Once this time is reached, lower levels of estrogen, progesterone, and other hormones result in the end of fertility. You are no longer ovulating, and you are forever in the postmenopausal period.


Every individual's menopausal experience is unique. Although you are no longer fertile, your hormone levels continue to decline with age. Hormones are responsible for transmitting signals through the bloodstream and tissues. These chemical messengers influence various processes, including metabolism, tissue growth, organ function, cognition, and mood. 


All bodies use estrogen, progesterone, and testosterone, among other hormones. Levels of estrogen and progesterone in your body slowly decline with aging in the postmenopause period. Testosterone can decrease as well.


Maintaining a healthy lifestyle during this time of transition is crucial for your health and can even prevent or alleviate some of these changes.



Health Concerns After Menopause (due to decreasing estrogen)

  • Osteoporosis - bone loss increasing risk for fractures

                           - Increase in LDL (bad cholesterol)

                           - Decrease in HDL (good cholesterol)

  • Cognitive decline
  • Genitourinary Syndrome of Menopause (GSM) (formerly vaginal atrophy) is the thinning of the vulva and vaginal tissue due to decreasing estrogen. When the vulvar tissue thins, the urethra is no longer protected and puts you at risk for chronic urinary tract infections (UTIs).

Not ready for a pregnancy? We offer telehealth visits for oral birth control pills and vaginal birth control rings. Intrauterine devices (IUD) and implants are excellent long-term options but we are unable to insert them via telehealth. We can provide education on all the various birth control options and the risks and benefits. Then if you desire an IUD, we can refer you to a provider and give you an alternative until you can get scheduled.
